00 an hour 22 hours ago 22 hours ago 22 hours ago CSI is accepting applications for a Case Manager to work with clients and families who utilize I/DD Waiver (Medicaid) program services.
After successful completion of a probationary period, some workdays may be completed from your home, so this is an ideal position for a self-starter! This position requires:
1.
A Bachelor's degree (BA/BS) in a human services field.
Recent graduates are encouraged to apply! The Case Manager:
Conducts monthly home visits - Some are in person, others are via phone.
Conducts quarterly day habilitation visits.
Schedules/holds/documents treatment plan meetings.
Provides linkage/referral to needed services.
Must be able to:
Proficiently operate a computer and have reasonable typing skills.
All documents are typed.
Send and receive e-mail.
Multiple e-mails are sent daily.
Use programs such as Microsoft Word, Outlook, Excel, and Adobe.
Work with numbers to analyze utilization and purchase of services.
Report to work on time and with few absences.
Meet deadlines/be organized.
Must have reliable transportation with current insurance, registration, and vehicle inspection and a valid driver's license.
Our office hours are Monday-Friday, 9am-5pm.
Some appointments may be required outside of these hours to meet client needs.
No weekends! Paid Time Off, holiday pay, mileage reimbursement and insurance benefits are available for this full-time position.
Rate of pay is based on demonstrated ability to complete I/DD Waiver case management duties in a satisfactory manner, as well as the number of cases one is able to efficiently manage.
